This emphasis on a low-flow state (peripheral hypoperfusion )underscores the basic pathophysiology in that type of shock most commonly encountered. Efforts to restore blood flow, by restoration of blood volume, by redistribution of perfusion and oxygen delivery, or by improvement in myocardial contractility will in most situations correct the shock state. However, septic shock usually involves a high-flow state, and factors other than blood flow become critical. Recent work has focused of metabolic change and cell injury, as well as on changes in the body's immune redponse, in determining the outcome of shock. Humoral and cell-mediated phenomena are especially important factors in septic shock.
